Title: Family Stabilization Case Manager
Reports to: Stabilization Manager
Hours: Full-time, non-exempt, days, shared on-call
Salary Range: $23 - $27/hour

Job Summary: The Family Stabilization Case Manager is responsible for providing counseling and case management to families while in the Shelter Program at Family Promise of Greater Rochester (FPGROC), as well as for 1 year after transitioning into permanent housing to ensure continued stability through the Aftercare & Stabilization Program. FPGROC is a nonprofit organization that assists families at risk of or experiencing homelessness to achieve sustainable independence by providing person-centered, holistic services in collaboration with the greater community.
Program Responsibilities
Interview prospective emergency shelter families. Meet with clinical team to assess and/or review potential families.
Assist families through a trauma-informed, client-centered approach in setting and realizing goals with counseling, advocacy, and resource referrals.
Maintain consistent communication with families via text, phone, and email to monitor progress in securing housing, as well as in-person contact at least once per week.
Maintain HMIS, enter HAT information, and place families on coordinated access waiting list.
Ensure families’ compliance with guidelines of the program, including bi-weekly apartment inspections.
Manage and initiate professional relationships with area property managers/landlords to facilitate access to adequate and affordable housing.
Meet with clinical team weekly to report on progress of families and share concerns.
Share on-call coverage for nights/weekends (approximately 2 weeks per month).
Maintain contact with property manager of shelter apartments to discuss maintenance and other issues that may arise. Submit maintenance requests through the online portal.
Continue case management support with housing, employment, finances, and advocacy for 1 year after transitioning from shelter into permanent housing to help maintain long-term self-sufficiency.
Administrative Responsibilities
Document interventions and services provided through use of an internal database and Google drive.
Maintain knowledge of Monroe County resources and provide appropriate referrals to address barriers to stable housing and employment.
Assist with ongoing continuous quality improvement to enhance program operations, including problem solving and helping to develop program systems or procedures.
Participate in ongoing education and relevant trainings or meetings to develop professional skills.
Attend all meetings relevant to the position, i.e. monthly staff meetings, coordinated access meetings, landlord engagement group, other committee meetings as needed.
Other duties as assigned.
Qualifications
Bilingual (Spanish/English).
Bachelor Degree in Social Work or MSW; Master's Degree preferred OR experience commensurate with education.
One to three years of related experience required.
Detail oriented and ability to perform organizational duties with a high level of accuracy.
Support and contribute to a creative, collaborative and respectful environment that promotes teamwork.
Demonstrates the necessary attitudes, knowledge and skills to deliver services from an anti-racist mindset, using a client-centered, non-judgmental, strength-based approach.
Ability to maintain confidentiality in regards to client information and records.
Ability to document work in a timely fashion.
Proficiency and experience with Google nonprofit suite.
Some lifting possible when helping families move into their own apartments.\
Must have valid driver’s license and willingness to use personal vehicle to transport if necessary (reimbursed for mileage).
